Needed Items to Apply for Assistance for ENTIRE household :
- Adults - Current Indiana "Picture" ID indicating them as a resident of Howard County and Social Security cards.
- Children - Birth Certificates and Social Security cards in which the adult applying has legal and physical custody or Printout from FSSA showing this information
- Proof of income and receipts for entire household for the last 30 days
- Current utility bills.
- Proof of Food Stamps (if eligible)
Shopping Dates (Tenative)
Sunday December 19 through Thursday December 22, 2012
Eligible households MUST shop on their assigned date and have current Indiana/Howard County ID. No children will be allowed on application or shopping dates
